Enrollment Fee

• $90/month
• Includes wellness visits, routine screening, urgent care visits, office procedures like joint injections and skin biopsies, and management of chronic conditions. 
• Labs and medications available at a low additional cost

• $50/month or $40/month for dependents of an enrolled adult
• Includes well child visits, routine screening, sick visits and management of any chronic issues.
• We do not stock vaccines but can help get them arranged for you

• $80/month for spouse/domestic partner of an enrolled member
• $40/month for each child of an enrolled member
• $250 max per family

While we are not able to bill Medicare insurance, we are now able to see people with Medicare insurance. Membership benefits and pricing are the same as listed above.
